OpenAI’s No. 2 Steps Back as Chronic Illness Collides With High‑Stakes AI Race
OpenAI’s second-in-command, Fidji Simo, is stepping away from day‑to‑day leadership just as the company pushes toward a potential IPO and intensifying competition in AI, forcing one of the industry’s most influential operators to choose recovery over one of tech’s most powerful jobs.
Early illness and rise at OpenAI
Simo was diagnosed with postural tachycardia syndrome (POTS), a chronic neuroimmune disorder that can cause dizziness, fainting, and rapid heart rate, in 2019. She joined OpenAI’s board in 2024 and, in May 2025, became CEO of Applications, a new No. 2 role consolidating product and business operations under her while CEO Sam Altman focused on research, compute, and safety.
Medical leave in April and leadership reshuffles
In April 2026, Simo told staff she was taking medical leave after a severe flare‑up of her chronic illness and that COO Brad Lightcap would move to a “special projects” role while CMO Kate Rouch stepped down to focus on cancer recovery. During her absence, OpenAI president Greg Brockman was put in charge of product and super‑app efforts as other senior leaders took on expanded business responsibilities.
By mid‑May, Brockman formally gained control of product strategy and scaling in a reorganization meant to align around a “single agentic platform” and merge ChatGPT and Codex into one unified AI agent experience.
July announcement: stepping down to advisor
On July 9, Simo told employees she would leave her full‑time role and become a part‑time advisor, saying the past three months had shown “the road to recovery would be much longer and more complex than I had anticipated—and that I needed to focus on it fully.” She described living seven years with a “disabling disease that still has no cure.”
Altman responded publicly on X: “i am really sad about this and very grateful for all fidji has done for openai… we all wish her the best for a speedy recovery. this sucks.”
Strategic fallout and ongoing pressure
Simo had been seen as a likely successor to take on more responsibility once OpenAI went public, making her exit “a real vacuum” for Altman to fill as the company eyes an IPO and chases Anthropic in enterprise AI. She had led a shift away from side projects like the Sora video app toward productivity and coding tools such as Codex to better compete with Anthropic’s Claude Code.
Brockman, who assumed product responsibilities during Simo’s leave, will now “keep heading up product at the company,” consolidating power among OpenAI’s co‑founder as these changes unfold.
